How to Become a Patient of the Leamington & Area Family Health Team

Applications are being accepted from residents of Kingsville, Leamington, Wheatley and Pelee Island who do not have a physician.
Becoming a rostered patient with the Leamington & Area Family Health Team is a three step process:

Step 1:
Applicants complete the application form. A single form may be completed for an individual applicant or for a family. This form will include some information about your medical history and your current medications.

Step 2:
A 3-part patient roster form is completed for each member of your family who is 16 or older. Youth applicants aged 16 or over must sign their own roster form. Children under 16 are included on their patient’s form. Applicants are also required to sign a Patient Declaration form.

Step 3:
You will be contacted by the office to schedule an appointment with your Physician or Nurse Practitioner that will be your primary health care provider. At this appointment, you and the primary health care provider will have an opportunity to discuss your health care needs and determine if your roster form will be processed.

Once you become a rostered patient, you will be given a copy of the roster form. The first copy of the form is submitted to the Ministry of Health and Long Term Care and the third copy is retained by the Family Health Team.

An applicant becomes a patient of the Leamington and Area Family Health Team upon receipt of a Physician signed roster form.
